As a privately owned outpatient Physical Therapy clinic, we accept referrals from all providers. Referring a patient is easy, just fax over the order and patient demographics and we’ll will contact the patient to get them scheduled. Or, give us a call to schedule the patient and then fax over the order!

For Tricare patients, please submit for authorization through the Tricare website.
